The International Association for the Study of Pain defines failed back syndrome as lumbar spinal pain of unknown origin that either appears after surgery for spinal pain that originated in the same topographical place or persists despite surgery. The pain may develop following surgery, or the surgery may worsen pre-existing discomfort or fail to effectively relieve it. Factors that may lead to FBSS include

Causes

  • Preoperative factors Such as obesity, pre-existing medical conditions, psychological issues, and chronic habits that affect health.
  • Surgical factors Such as choosing an inappropriate surgical technique, treating more than one spinal level, or surgeon error.
  • Postoperative factorsSuch as scar tissue formation, spinal infection, or further degeneration of the spinal column. 

Symptoms

Other factors that may contribute to FBSS include:

  • Recurrent spinal disc herniation.
  • Persistent pressure on a spinal nerve.
  • Altered joint mobility.
  • Preoperative psychiatric disorders.
  • New onset spinal pathology.
  • Trauma or stress from adjacent muscles. 

TREATMENTS

Treatment options for failed back syndrome include antiepileptics, non-steroidal anti-inflammatory medications, oral steroids, antidepressants, and opioids.

You may enhance your posture and gait, as well as your physical function and muscular strength, with physical therapy.

The symptoms associated with unsuccessful back surgery may be treated with epidural steroid injections (ESIs) and spinal cord stimulator trial/implant.
